At approximately 11:00 a.m. on February 21, 2012, an employee was working on a b racket scaffold changing out a light bulb in a boiler tune. Employee #1 stepped over to next section and fell through where a plank had moved off of its support . Employee #1 fell approximately 19 feet to the bottom. The employee received mi nor bumps and bruises to the head and arms.
