At approximately 7:30 a.m. on July 17, 1997, Employee #1, of Midwest Fabricating Company, was using a Bliss power press brake to bend U-clips. He was using a 8 to 10 ft long wooden stick to hold the parts down. Employee #1 was trying to pick up a finished clip with his left hand when the ram came down, amputating his left index finger. Employee #1's supervisor had instructed him not to use the pull-back devices that day, due to the nature of the work. The employer was cited for a serious violation of T8CCR 4214 (b)(9).
