Employee #1 was working as a clerk at a gas station in Vallejo, CA. He and a coworker were assigned to clean and remove water from a manhole using a 5-gallon bucket as a scoop. As he stood up in the manhole to haul out a bucket of water, a motor vehicle drove over the manhole and struck Employee #1 in the head. He sustained a head laceration and a neck injury, and was transported to the hospital.
