At 10:00 a.m. on January 26, 1994, Employee #1, a 43-year-old machinist, amputated his left thumb while operating a power-operated press. Employee #1, a newly trained employee, was pressing one and one-half inch flat washers into a concave shape when his thumb entered the unguarded point of operation. Employee #1 sustained an amputation of the distal phalanges of the left thumb. The machine involved in the accident was a Rousselle 3G Punch Press. The shop foreman had performed the training and initial observation of the machinist about one and one-half hours prior to the accident. | |||||||
