Employee #1 had a flat on the inside left rear dual wheel of a 1966 Ford truck (DMV 1B83887). He drove to a tire shop to get it fixed. When they saw that the wheel was a split rim, they informed him that they didn't service that type of wheel. Employee #1 asked if he could used their air hose to add air to the tire. They said yes, but warned him to be careful of the split rim wheel--they are dangerous. Employee #1 began adding air to the tire and in approximately two seconds it exploded. Employee #1 was blown approximately 10 feet away from the rear wheel. He sustained a concussion and was killed.
