Employee #1, who was newly hired, was aiding in stacking structural column pieces. Employee #1 was supervised by a forklift driver. The forklift driver was supervised by the lead man, who was away from the site. This type of work is infrequent and required extra preparation regarding methods of restraining cargo and avoiding danger zones. After the stack collapsed, Employee #1 was found lying on the ground. He was in a danger zone, and was taken to the hospital and admitted for two to three days. Employee #1 was treated for a head trauma injury. Available cargo restraint equipment was available, but was not used and no training was given in its use. | |||||||
