Employee #1, a semi-truck driver, was in line to have his container loaded onto a ship. Thinking the prior container was safely aboard, the checker had signaled Employee #1 to come forward when there was a failure in the crane's electrical system that caused the container to fall. The container hit the sill beam of the crane, bounced, and hit the front bumper of the semi-truck. The impact of the container hitting the bumper and then the ground caused Employee #1 to be tossed around in the cab of the truck. He sustained back injuries.
