At 7:56 a.m. on December 11, 1992, Employee #1 was attempting to hook an extension cord to a Sky Climber, which was approximately 60 feet high on the roof of the east precipitator. The roof of the precipitator was covered with visqueen to prevent moisture from entering. The wind was blowing the visqueen up beyond the roof edge. Employee #1 stepped onto the visqueen beyond the roof edge and fell approximately 45 feet to the second level floor. Employee #1 sustained a broken arm, facial lacerations, broken dentures, and lost his prescription glasses. The area had not been identified as a fall hazard on the fall protection work plan; thus, standard guardrails or the equivalent had not been installed around the perimeter.
