Employee #1, of McClean Steel Corporation, was using a button punch tool on a previously installed metal deck. He was walking backward on the second-floor level when he tripped on a concrete floor form. Employee #1 fell through a 12 ft by 20 ft floor opening for an entry lobby balcony and sustained injuries that required hospitalization. There was no guardrail, cover, or any other safety measure that would have prevented this accident. The employer was cited for a serious violation of safety orders.
