At approximately 4:10 p.m. on August. 8, 1991, Employee #1, of Tru-cast Concrete Products, Inc., Corona, CA, was assigned to clean a batch mixing machine on a raised platform in Department #31. When he entered the machine, the main switch and disconnect had not been locked out. A gust of wind blew the access door shut, energizing a switch on the access door that stops the machine when the door is opened. The mixing machine started and Employee #1 was crushed and killed.
