At 1:32 p.m. on December 23, 1991, Employee #1 was standing next to the open lid of an asphalt tank, observing the asphalt as was heating. A light breeze carried the rising vapors downwind from the tank toward where a torch was being used to heat oil lines. The asphalt vapors ignited and Employee #1 sustained burns to his face, neck, hair, and upper arms. He was treated at the site and then flown by helicopter to the local burn center.
