At approximately 7:30 a.m. on May 20, 1993, Employee #1, a journeyman carpenter, amputated the tip of his left index finger with a Skil saw he was operating. He was transported to Hollywood Presbyterian Hospital for observation and treatment. Investigation indicated that the saw may have kicked back when his hand was in or near the line of the saw. It was his second day on the job but he had received safety instructions from the employer. The saw was not examined immediately after the accident and the depth setting and condition of the guard was not determined. The exact cause of the accident could not be ascertained. | |||||||
