At approximately 10:10 a.m. on August 29, 1990, Employee #1 was on a ladder about 25 ft above the ground, painting the outside of an apartment building. The employer stated that the ladder suddenly collapsed, throwing Employee #1 to the ground. Employee #1 suffered contusions and strains; he was taken to Seton Medical Center in Daly City, CA, for treatment. The ladder had not been not tied or otherwise secured.
