At approximately 9:40 a.m. on April 23, 1992, Employee #1 was working on level 4 of a job site, 19 ft above level 3. There were no guardrails or nets protecting the employee, who was not wearing a safety belt. In addition, no adequate risk control was maintained under any immediate competent supervision. A 15 in. long rebar fell from level 8, struck Employee #1's hard hat, dented it, and knocked him off balance. Employee #1 fell 11 ft to the plywood cover of a duct. He bounced off and fell 8 ft to level 3. Employee #1 was hospitalized with head, neck, back, and stomach injuries. A serious violation of T8CCR 1669(a) was observed and cited. | |||||||
