| A fuel and ash technician was in a coal yard breaker house working on an ITE K600 480-volt, 300-ampere circuit breaker. As the employee was racking the circuit breaker back in, an electrical fault occurred. He sustained second- and third-degree burns to his right arm, shoulders, back, neck, and face. He was hospitalized for his injuries. His employer required a switching jacket, ultra-violet-rated face shield, gloves with leather protectors, and rubber insulating sleeves. However, the employee was not wearing the proper personal protective equipment. | |||||||

| An electrician with more than 30 years experience was placing an electric meter into a deenergized plug as part of the installation of a new service at a warehouse. He was standing in front of a large 480-volt service cabinet. Apparently, an electrical fault occurred in the service cabinet. (The cause of the fault was not determined.) The electrician was burned by the ensuing electric arc. He was treated for first- and second-degree burns on the left side of his face and neck near his ear. | |||||||

| A power line crew was working from an aerial lift on an overhead power line. The two-person crew had placed an extension arm on the crossarm on a utility pole and was preparing to remove the rubber insulating equipment from the conductors. One of the power line workers was tying off a conductor that had been placed on the extension arm when the end of the crossarm broke, allowing the extension arm to swing down and come to rest against another crew's aerial lift bucket. While the extension arm was falling, two of the phase conductors on the power line faulted. The ensuing electric arc ignited the shirt of the power line worker who had been tying off the conductor. His coworker lowered the aerial lift bucket, and the injured employee was removed. Emergency medical services arrived in 5 to 10 minutes and transported him to a hospital. The employee was later airlifted to a regional burn center, where he was hospitalized for treatment of burns to his face, neck, and chest. | |||||||

| An electric utility employee was using a paintbrush to dust a motor control center. The 50-millimeter-wide paintbrush had a metal band binding the bristles to its handle. To perform this job, the employee had opened the circuit breaker to the motor; however, the line side of the breaker was still energized. The opening between the line side of the circuit breaker and the grounded frame was about 38 millimeters wide. When the employee put the brush into this opening to dust inside, an electrical fault occurred. The ensuing electric arc and fire burned the employee's right hand, arm, and shoulder, and his face, back, and head. The burns ranged from first degree to third degree. | A power line worker was assigned to replace a knife switch at a substation. He was to unbolt the switch after a coworker removed the energized leads to the switch to deenergize it. To perform this task, the power line worker had exchanged a hand ratchet wrench for a hydraulic impact wrench. This reduced the approach distance between the switch and the tower to less than 25 millimeters. The power line worker started to remove the switch before it was completely deenergized. As the power line worker was unbolting the switch, the impact wrench contacted the tower, and an electrical fault occurred. The ensuing electric arc burned the power line worker. He sustained first-, second-, and third-degree burns above his waist, including burns to his head and hands. He was hospitalized for his injuries. | |||||||

He was trying to identify conductors running from the switchgear to a pit where his crew leader was working. An electric arc occurred (the cause of which was not reported). The power line worker was burned on his right arm, his chest, and his face.
