EMPLOYEE #1 WAS TESTING A DIE TO BE USED FOR REMOVING FLASH AROUND PLASTIC FOOD SERVING DISHES IN A RESEARCH AND DEVELOPMENT FACILITY. THE PART WAS HAND FED INTO THE DIE AND APPARENTLY POSITIONED IMPROPERLY. EMPLOYEE #1 ATTEMPTED TO REPOSITION THE PART AFTER ACTUATING TWO HARD TRIPS. THE PRESS CYCLED, AMPUTATING HIS FINGERS. HE WAS WEARING PULLLBACKS AT THE TIME OF THE ACCIDENT, BUT THEY WERE NOT ADJUSTED FOR HIM BEFORE HE WAS ALLOWED TO OPERATE THE MACHINE. | |||||||
