At approximately 2:30 p.m. on December 18, 1989, Employee #1 was attempting to unhook a choker from the crane hook, while standing with one foot on the mid rail of the wire rope guard rail and the knee of his other leg on the top rail. A welded washer that was separating the wire rope broke free of the weld causing the line to go slack, this caused Employee #1 to fall over the top rail to the ground. He sustained the following injuries: two broken wrists, fractured pelvis, and a bruised arm. Employee #1 was hospitalized. | |||||||
