At 4:15 a.m. on September 2, 2020, Employee # 1 was filling up fuel cans at a fu el station with a forklift. He fuel cans on the body of the forklift and then we nt to reach for the cans. He accidentally activated the forklift boom lever, low ering the boom on top of himself, pinning him to the truck. Employee # 1 was pin ned and killed.
