At 11:00 a.m. on December 11, 2019, an employee was sanitizing the pasta extruder with a chemical cleaner, Vigil-Quat that contained ammonia and CFT Superfoam that contained sodium hydroxide and sodium hypchlorite. The employee was talking with a coworker, turned to walk down the stairs, and fell face first onto the floor below the stairs. The employee lost consciousness and sustained a spine injury. The employee also developed pneumonia and was killed.
