At 11:17 p.m. on November 5, 2019, an employee was working for a temporary staffing agency. He had been sent to work in a warehouse. He was operating a Crown order-picker. While he was elevated to approximately 3 meters (10 feet), he backed the equipment with products on the picking deck. The boxes struck a racking stanchion and shifted into the operator station, striking the employee. He was crushed between a box and the operator cab. He was killed.
