Between 7:00 a.m. and 9:00 a.m. on June 20, 2001, Employee #1, a heavy equipment mechanic and tire serviceman working for Witten Logging Company, was killed while repairing a log skidder tire. The tire was being held in a vertical position by a chain when it fell to the horizontal position, pinning Employee #1 between the rim and the tire bead, asphyxiating him. The accident occurred at a remote location in the San Bernardino National Forest, 7.1 miles northwest of Fawnskin on U.S. Forest Service property.
