At 8:00 a.m. on March 11, 2019, Employee #1, employed by a roofing contractor, w as engaged in roofing repair work on the roof of a two-story multifamily residen tial building. He had accessed the roof while wearing his harness and lanyard. As he crossed the roof toward the anchor point so he could attach his fall prot ection equipment, he slipped and fell, striking a guardrail The employee fell 2 1 feet. He was transported to the hospital and admitted for treatment of multip le injuries, including fractures to an elbow, his collarbone, and a finger.
