At 4:15 p.m. on August 2, 2019, an employee was being trained to operate a punch press machine. The trainer left the employee alone and had him use the wrong eq uipment, such as a foot pedal. The employee was hospitalized to treat an amputat ion to the tip of the left thumb and to the right pointer finger.
