At approximately 6:00 a.m. on July 18, 2019, an employee was operating an electr ic pallet jack #1110 (Crown PE 3000 SN:6A294328) along the speed aisle, which ru n perpendicular to numbered aisles. The speed aisle has the right away to the nu mbered aisles. While coming to the intersection of the speed aisle, the employee did not yield causing the PIV to collide. The employee fell off in the path of the forklift, which ran over his lower left leg. The employee's leg was fractur ed. The employee was hospitalized for these injuries.
