At 12:30 p.m. on January 24, 2019, an employee was standing on a 12 foot A-frame ladder. The employee tried to climbed up on one of the last e-rungs of the ladd er to access the ceiling through a hatch. The employee fell off the ladder. The employee sustained back and head injuries and was hospitalized.
