At 8:30 a.m. on May 2, 2019, an employee had just accessed an interior parapet w all utilizing a portable ladder. The employee was travelling from one roof surfa ce to another roof surface and was reaching for a second portable ladder that wa s located on the other side of the parapet wall. The ladder did not extend 3 fe et above the wall as required. The employee fell approximately 6 feet from atop the parapet wall while reaching for the ladder. He suffered fractures to the arm , wrist and leg, and lacerations which included a head wound and suffered loss o f consciousness, requiring hospitalization.
