At approximately 1:30 p.m. on April 15, 2019, an employee maintenance supervisor who was trying to link a security camera to its base. The employee used a 20 L ouisville extension ladder to access 12 , 6 mezzanine. The employee was not car rying anything while climbing ladder. The employee was working in the egg packag ing building (mezzanine area for maintenance, sanitation and chemical room). The employee was climbing an extension ladder to access the mezzanine and was head high on the level of the mezzanine (approximately 12 feet, 6 inches) when the ex tension ladder started to slide out from under the employee. The employee susta ined injuries to his chest, ankle, back and head and was hospitalized for these injuries.
