At approximately 1:00 or 1:10 p.m. on April 12, 2019, Employee #1, a 53-year-old English-speaking employee, was working at a firm that made plastic bottles. He was working on the Line Number 29 Proco bottle transfer unit. The unit had becom e jammed with bottles. The employee was trying to clear the jam. The employee al legedly removed the interlock guard to reach in to remove the jammed bottles. At that point, the tip of his finger was amputated. It was not certain whether the employee's finger was cut on the sharp edge of the plastic or between the cylin der back plate and the tooling back plate, which was pneumatically controlled. H e was not hospitalized. The employer did not require lockout for this activity. Although the unit was equipped with a guard, the side of the guard was not adequ ately fastened.
