At approximately 3:30 p.m. on April 1, 2019, Employee #1 was operating the Angle Broom Machine and noticed that a bundle of broom fiber bristles had not loaded correctly in the feed table. He reached into the feed table with his left hand to straighten the bundle when the machine cycled. Employee # 1's distal tip of the left pinky was amputated when the machine cycled.
