At 11:20 a.m. on March 28, 2019, an employee placed a piece of wood between a to te full of waste oil and a forklift as a spacer and was making an adjustment to it when a coworker moved the forklift forward. The employee's left hand thumb w as caught between the forklift forks and the piece of wood, amputating it. The employee was hospitalized.
