At 3:35 p.m. on March 18, 2019, Employee #1 was cutting small pieces of PVC with a table saw. He was acting as a supervisor, training a temporary employee. The saw blade caught Employee #1's right index finger. The employee reached to the piece being cut before it had been fully cut. This action caused the piece to bind, pulling his right hand into the blade. The employee had been using a pushing tool provided by the saw manufacturer. The employee sustained a partial amputation of the tip of the index finger, with two other fingers affected. The employee was hospitalized.
