At 12:30 p.m. on March 20, 2019, Employee #1 was looking at a conveyor belt to s ee why it was not moving and looking for a board that jammed the belt. The emplo yee placed his gloved hand on the top of the belt/pulley guard. The back of belt /pulley was not guarded. Glove finger tip was pulled into the pulley when the b elt started moving again. The employee sustained a laceration, resulting in an a mputation. The employee was hospitalized.
