At 7:15 a.m. on March 14, 2019, an employee was operating a Triangle produce bagging machine located in the packaging area and was processing carrots that were being fed by a conveyor system. The employee reached under the plexiglass guard doors and up to the blade system, amputating his left hand's index fingertip. The employee was hospitalized.
