At 1:15 p.m. on February 12, 2019, Employee #1 was being trained by a coworker on operating the foam impactor machine. The two employees had loaded the impactor and the coworker proceeded to use the switch to close the platens of foam impactor #2 at a time Employee #1 reached to make an adjustment to a 1.125 spacer. Employee #1's fingers were on top of the spacer moving it, when the coworker operated the switch to close the platens. A platen closed and pinched the ends of three fingers on Employee #1's right hand. Employee #1 was located out of view from the coworker, who was at the machine's operating switch. Employee #1 sustained a crushing injuries to the ends of the right second, third, and fourth fingers that resulted in amputation to the ends of the second and third digits. The employee was hospitalized.
