At 2:30 p.m. on January 28, 2019, an employee was monitoring the iron oxide being filled into a railcar and climbed the stairs to check the level of the bin. The employee determined there was a clog at the rotary valve and removed the guard cover to watch the rotary valve move slowly due to being clogged with iron oxide. The employee used a screw driver to remove some of the material and placed his hand by the guard opening to see if he could feel the air movement. The employee caught his fingers between the wall of the rotary valve and the moving blades, amputating his fingers from his middle and ring fingers to the first knuckle. The employee was hospitalized. | |||||||
