At 3:00 p.m. on January 17, 2019, Employee #1 was working as a machine operator, doing a variety of tasks. The employee was working with two other employees (another tostada operator and a baker) operating the tostada line. Employee #1 removed the sheet metal covering that hung on the side of the tostada fryer to clean out the tostada shells that had fallen to the bottom of the fryer. The machine was in operation and Employee #1 reached in with his hand and made contact with either the chain and sprocket or the moving parts of the basket (Employee #1 was unsure and event happened so fast) amputating the tip of his right thumb. The employee was hospitalized. | |||||||
