At 12:00 a.m. on September 14, 2018, an employee and a coworker were rewinding t oilet tissue paper in the machine restricted area onto an embossing rollers of a Fabio Perini Constellation, unit #33106. Another coworker was jogging the embo ssing rollers from the exterior of the cage. The employee and coworker were pull ing fresh toilet tissue sheets into position while another coworker advanced the rollers. The employee reached in to smooth paper as it entered the rollers. Th e employee sustained a partial left thumb amputation and a laceration to his pal m. The employee was transported to the hospital for treatment of his injuries.
