At 9:45 p.m. on December 19, 2018, an employee was cleaning a Janssen Cookie Molding Machine Model # FM 128, after the daily production run was completed. The employee opened the hopper's lift grid guard and was removing the left over dough when the machine started and caught three fingers from his left hand in the machine rollers and crushed them. The employee amputated his index finger and had surgery performed on the other two. The employee was hospitalized.
