At 8:45 a.m. on November 16, 2018, an employee was attempting to retrieve a tele vision from the upper rack of stock room. The employee failed to utilize a fall protection lanyard and body belt of cherry picker. He threw the lanyard and belt over the rear of the machine. At some point during ascent or initial descent th e lanyard got stuck in the chain of the cherry picker that controlled the platfo rm of the machine. The employee utilized a mallet and knocked the lanyard lose w hich resulted in the machine's platform falling approximately 8 to 10 feet. The employee broke his right ankle during the fall after contact with the concrete g round.
