At 9:45 or 9:47 a.m. on Saturday, September 8, 2018, an employee was working at an elementary school. He was a member of a grounds crew. Three coworkers were wi th him. It was not clear whether his supervisor, the maintenance supervisor, was among them. The grounds crew was delivering and spreading mulch at the elementa ry school. They had been allocated a 2007 Chevrolet C6500 dump truck, with truck license number 1308888. The crew also had hand tools, such as shovels and a whe elbarrow, to execute the task. The employee was standing at the rear of the dump truck, directing his coworker, the driver. The employee was directing the drive r of the dump truck to raise or lower the truck bed and guiding the driver when the vehicle when backing up. The driver was raising the bed of the truck. The em ployee had his hand placed on the tailgate of the dump truck. As the bed went up , the employee grabbed the tailgate and attempted to close it. He sustained a cr ushing injury to his right index finger. He also sustained a fracture to that fi nger. He drove himself to Kaiser Permanente for medical treatment using his own medical insurance. He was hospitalized. The school district notified the Divisio n of the injury via telephone call at approximately 1:16 p.m. on September 10, 2 018. The employer notified the Division within eight hours of learning that thei r employee was hospitalized greater than 24 for other than medical observation. A representative of the Division reported to the scene of the incident at approx imately 11:26 a.m. on November 14, 2018 to conduct an investigation.
