At 5:00 a.m. on October 28, 2019, an employee was troubleshooting a TRI--STAR CT 28 Gas Heated Flatwork Ironer/Folder Machine and had removed a guard that was over a gear box but did not lock out the machinery. The employee fractured his right hand's index finger when it was caught in the chain. The employee was hospitalized.
