At 1:30 p.m. on October 11, 2018, an employee was operating a switchbox former machine when a jam occurred with Machine #5. The employee did not shut down the machine when he reached under the guard to clear the jam and caught his right hand's thumb, partially amputating it. The employee was hospitalized.
