At 9:00 p.m. on October 17, 2018, Employee #1 was conducting a visual inspection of the liner plates.While conducting the visual, Employee #1 placed his right hand in between the roller and machine grabbing his gloves and crushing and fracturing his hand. The guard had been removed. The employee was hospitalized.
