At 6:11 a.m. on October 3, 2018, an employee was straightening a piece of mylar after noticing that it was twisted at the tip and put her finger through an opening in the guard. The employee then activated the cutting blade and caught her fingernail, removing it from her finger. The employee was transported to the hospital; however, hospitalization was not required.
