At 11:15 a.m. on October 1, 2018, an employee was working on a retract cylinder that was frozen. The employee pushed on the cylinder, freed it, and it struck h is right hand's middle finger. The employee amputated the tip of his middle fin ger above its first knuckle. Hospitalization was not required.
