At 11:00 p.m. on September 13, 2018, an employee was clearing a blocked photo eye on the spline remover and entered the guardrail while the remover was running and other equipment was not isolated or locked out. The employee's right hand came into contact with the remover head and was amputated at the wrist. The employee was hospitalized.
