At 5:20 a.m. on June 18, 2018, Employee #1 was performing function tests and air leak checks on a newly assembled vacuum, Model number 9215LDV. As the employee was running his left hand around the blower motor to check for leaks, his finger came in contact with the spinning fan of the blower motor. He sustained a partial amputation to his left hand index finger.
