At 10:00 a.m. on August 20, 2018, Employee #1 was demonstrating how to clean and replace a pool filter for a coworker. The Pool filter cover blew off and struck the left hand of Employee #1, causing a amputation of left thumb and ring and middle fingers. The employee was hospitalized.
