At 10:30 a.m. on August 7, 2018, Employee #1 was setting up a packaging machine. The employee needed to advance the packaging film, so he reached under the guard and up to the sealing cutting area to jog the machine and manually advance the film. He struck his finger on the film cutting knife and amputated the tip.
