At 5:30 a.m. on August 15, 2018, Employee #1 was being trained by a coworker to operate the winder. Operators were working on the Rossendale Line #1 Roll Winder to change the rolls and observe the operation as the material was wound onto the rolls. On the second or third roll, the employee noticed that it wasnt being wound tight enough, resulting in another scrapped roll. Employee #1 observed that the material was not lying flat on the conveyor belt just before it was rolled onto the winder. She put her right hand down onto the material in an attempt to get it to lay flat before it was rolled. In doing this, the employee's hand was immediately adjacent to the in-going nip point and it was caught and pulled in. Her coworker accessed a safety mat to stop the machine, but Employee #1 sustained crushing injuries and lacerations to her right hand, requiring surgery.
